Counselling Agreement Under 18's Parent and Carer Agreement
Please read carefully and ask any questions you may have before signing.
Welcome to Rosewood Therapy. This agreement outlines the terms and conditions of the counselling relationship between your child (the client) and me, Catherine, your counsellor. I hold a current Enhanced DBS check and full therapeutic insurance as well as being an accredited member of the NCPS.
By signing this agreement, the parent or legal guardian confirms that they have read and understood the terms and are providing consent on behalf of the child. This agreement covers both face to face, remote and other modes.
The counselling sessions aim to support clients emotional health, personal development, and coping strategies. Sessions will be age-appropriate and child-centred.

Therapy Approach
I work integratively, with a solid foundation in person centered therapy, tailoring the therapeutic approach to suit individual needs.
Session Details
Session Duration: 45 minutes
Session Frequency: Weekly unless otherwise agreed
Location: Will be confirmed during initial call.
Session Fee: £60 per session

Cancellation Policy: Cancellations made with less than 24 hours’ notice will result in being charged the full session fee.
Attending Counselling: Counselling works best when attended regularly. By agreeing to this, you commit to:
Your child attending scheduled sessions or informing the counsellor at least 24 hours in advance if they cannot attend.
Respecting the counselling space and your own emotional boundaries.
Parental Involvement
You are encouraged to support the counselling process. While respecting the child’s confidentiality, I may offer general feedback or discuss progress when appropriate and with the child’s consent.
Confidentiality
Confidentiality is fundamental to therapy. However, there are specific circumstances in which confidentiality may be breached. I will explain when this may happen to your child at the start of the sessions.
- If I believe your child or someone else, is at serious risk of harm.
- If there is disclosure of a child or vulnerable adult being at risk.
- If required by law (e.g., under court order, or relating to terrorism, money laundering, or drug trafficking).
- Disclosures may also be made in supervision, where the childs identity is protected.
In the case of children and young people, confidentiality will be maintained, but you will be informed if there are safeguarding concerns.
Record Keeping
Brief notes are kept after each session for clinical purposes. These will be anonymized.
Records are stored securely in line with GDPR and Data Protection Act 2018.
You have the right to access your records upon written request.
I may discuss my notes with my supervisor. This will be anonymous and no identifiable information will be used. This is done to ensure that we are keeping your child safe.
Under the General Data Protection Regulations (GDPR) 2018, your child has certain rights regarding their personal information. As a parent or guardian, you are agreeing to the following on their behalf:
Access to notes: Your child has the right to see the notes taken during therapy. This can be done by making a formal request.
Amendments to records: You, as a parent or guardian, can request changes to your child’s records, such as correcting their name or address.
Records for clients under 18: If your child is under 18, their records will be kept for 7 years after they turn 18.
Safeguarding
Rosewood Therapy has a responsibility to safeguard the welfare of children, young people, and vulnerable adults.
If a safeguarding concern arises, I may need to involve external services such as Children’s Services or Adult Social Care. Where possible, this will be discussed with the client and parent or guardian, where applicable beforehand
Termination of Therapy
You or your child may end therapy at any time.
It is beneficial to discuss ending in advance, to allow for a planned and supported closure.
I reserves the right to terminate therapy if the working relationship breaks down or if it is not in the child's best interest to continue.
If a child is aged 16 or over, they can make this decision independently.
For those under 16, they may end counselling if they are assessed as Gillick competent, meaning they fully understand the process and its implications.
Children under 13 cannot legally consent to or end counselling on their own, and a parent or guardian’s involvement is usually required. However, the child’s views will always be taken seriously, and counselling will not continue against their will unless there is a serious safeguarding concern.
In all cases, the child’s wellbeing is our top priority and I will support the child in making this decision and preparing for a positive and supportive ending.
Online/Telephone/Other Counselling
If sessions are conducted online:
A secure platform will be used. I use Microsoft Teams.
Children should ensure they are in a private, quiet space during the session.
Technical disruptions will be managed fairly and professionally.
I ask that you and your child do not record our session, and I will do the same.
Cameras will be on to ensure a safe space for both.
Telephone counselling will only be completed on the given numbers.
