Counselling Agreement For Adults

Please read carefully and ask any questions you may have before signing.

Welcome to Rosewood Therapy. This agreement outlines the terms and conditions of the counselling relationship between you (the client) and me, Catherine, your counsellor. I hold a current Enhanced DBS check and full therapeutic insurance as well as being an accredited member of the NCPS.

 Therapy Approach

I work integratively, with a solid foundation in person centered therapy, tailoring the therapeutic approach to suit individual needs. 

Session Details

Session Duration: 50 minutes
Session Frequency: Weekly unless otherwise agreed
Location: Will be confirmed during initial call.
Session Fee: £60 per session

Cancellation Policy: Cancellations made with less than 24 hours’ notice will result in being charged the full session fee. 

Attending Counselling: Counselling works best when attended regularly. By agreeing to this, you commit to: 

Attending scheduled sessions or informing your counsellor at least 24 hours in advance if you cannot attend.
Engaging openly and honestly in the process.
Respecting the counselling space and your own emotional boundaries.

Confidentiality

Confidentiality is fundamental to therapy. However, there are specific circumstances in which confidentiality may be breached. I will explain when this may happen at the start of your sessions.
- If I believes there is reason to believe that you, or someone else, is at serious risk of harm.
- If there is disclosure of a child or vulnerable adult being at risk.
- If required by law (e.g., under court order, or relating to terrorism, money laundering, or drug trafficking).
- Disclosures may also be made in supervision, where your identity is protected.
In the case of children and young people, confidentiality will be maintained, but parents or guardians will be informed if there are safeguarding concerns.

Record Keeping

Brief notes are kept after each session for clinical purposes.  These will be anonymized. 
Records are stored securely in line with GDPR and Data Protection Act 2018.
You have the right to access your records upon written request.

I may discuss my notes with my supervisor.  This will be anonymous and no identifiable information will be used.  This is done to ensure that I am keeping you safe.  These notes are stored securely in a locked filing cabinet for a minimum of seven years.  Any electronic information is held securely within Rosewood Therapy's systems, in line with data protection regulations.

Under the General Data Protection Regulations 2018, you have certain rights. These are:

You are allowed to see your notes. This is facilitated by making a formal request.
Records are kept for 7 years after termination of therapy and then destroyed.
Having your records amended (change of name and address)
In the case of clients under 18, records are kept for 7 years after the child turns 18.
Under GDPR, you can request your data to be erased.

However there are exceptions to this.

In the case of counselling records, insurance companies and ethical bodies ask for records to be available for the period of time as outlined above.
By completing and submitting this form, you are agreeing to having your records kept for 7 years after the termination of therapy.

Supervision 

To maintain good practice, I may discuss our work with a professional supervisor. This helps ensure you receive the best possible support.
I will not share your name or any identifying details, so the supervisor will not know who you are.

Safeguarding

Rosewood Therapy has a responsibility to safeguard the welfare of children, young people, and vulnerable adults.
If a safeguarding concern arises, I may need to involve external services such as Children’s Services or Adult Social Care. Where possible, this will be discussed with you beforehand.

Termination of Therapy

You may end therapy at any time.
It is beneficial to discuss ending in advance, to allow for a planned and supported closure.
I reserves the right to terminate therapy if the working relationship breaks down or if it is not in your best interest to continue.

Online/Telephone Counselling

If sessions are conducted online:
A secure platform will be used.  Such as Teams
Clients should ensure they are in a private, quiet space during the session.
Technical disruptions will be managed fairly and professionally.

Try to find a quiet, private space where you feel safe to talk

If something goes wrong with the tech, we’ll find a fair solution

I will ask you not to record the sessions and neither will I

Telephone sessions will only be done on approved numbers

Emergency Contact
As part of our commitment to your safety and wellbeing, you will be asked to provide emergency contact details. This information will be stored securely and will only be used in the event of a serious concern for your immediate safety or wellbeing.

Wherever appropriate, your counsellor will aim to discuss the use of this information with you beforehand. It will not be used for any other purpose without your consent.

Rosewood Therapy is not an emergency service.

Complaints Procedure

If you are dissatisfied with the service:
1. Raise the concern directly with your therapist.
2. If unresolved, you may contact their professional membership body NCPS.
